ZD, try rolling back to an older video driver. I ran the 190.22 Nvidia drivers when I had my GTX260. The "latest and greatest" aren't always the best for an older card. Use Driver Sweeper
http://download.cnet.com/Driver-Sweeper ... 19868.html after you use the Windows uninstaller. Pic you posted is either a driver problem, or the card is dying. I hope it's just a driver issue.
Also, do you run the EVGA Precision Tool to control fan speed, and monitor card temps?
