Unless I'm missing something (and I might sure be with this stuff scattered all over nowadays) there is no model that has the headlight up top and the right windows?

Eyein has a sd60m widecab with the right windows. Not sure about the headlight situation. Everything else is a sd70 variant.

The Eyein12 SD60 wide cab pack has the correct windows and twin headlights over the centre of the cab windows. Other versions of the SD60M are available from eyein12 with headlights on the nose take care to get the correct one, although they are not particularly expensive.
Edit: $15 each version
