Goodwell was apparently caused by the Z train engineer's inability to distinguish color light signals (history of eye problems) and the conductor was apparently not calling signals...

It doesn't matter what signaling system you use if your engineer has deteriorating eyesight and neither crewman pays attention to what is going on. I'm not totally affluent to how signaling systems work but shouldn't an alarm have gone off somewhere that a train blew through a stop signal?

The dispatcher should have seen that, I would think.
